    Hi There,

    I need to create a question where the user has a choice to rate each item in a selection. They can only rate each item once and only choose the number once.

    Rate a selection example:

    What is your favourite fruit?
    Please rate the following (1 being the best - 5 being the worst)
    1 2 3 4 5
    Pineapple [ ] [ ] [ ] [ ] [ ]
    Banana [ ] [ ] [ ] [ ] [ ]
    Apple [ ] [ ] [ ] [ ] [ ]
    Pear [ ] [ ] [ ] [ ] [ ]
    Orange [ ] [ ] [ ] [ ] [ ]

    So answers may be:

    Pineapple 1, Banana 5, Apple 3, Pear 2, Orange 4
