    hi

    when users click "get assistance with this form", they get "person responsible has not entered contact information."

    how can i fix that?

    You have not provided any contact information in your account profile. Please go to 'My Account'->'Contact Infomation' and fill out 'Public Contact Information'. It would also be good to provide us with the other information on that page.
