I embedded my form into my site (http://design.webtools2go.com/customPages/our-services) and it all looks fine. Except that the selector on top should change the options below it. It works like magic in the form builder, but not when I embed it into an external site. BTW it is embedded with the html editor of TinyMCE.
It looks as though a section of the css is not coming along with the rest of your code. Try republishing your form and adding the republished code back in to your hosted site. We can reassess if that doesn't work.
Also, just fyi, the file upload is going to cause errors with form submission at your subscription level. It shouldn't be causing the issue you're reporting here, but will cause other issues if you take the form live without upgrading from the free plan.
