    How do I take a form/questionnaire that I have created and embed it in a word doc to use?

    Why do you need it in a word doc? The whole point in using FormAssembly is to have it be web based.

    Let me rephrase that - If you plan to print the form, then you would need to build the form in Word. Now if you like the layout of the form you could do the following:
    You could simply do ctrl + a to highlight everything in your form, then ctrl +c to copy it to the clipboard. You then do ctrl + v to paste it into Word. I just tested it and it does work but your formatting may be messed up.
